APPLICATION STATEMENT

I certify that all information I have provided in order to apply for and secure work with Communications International (Ci) is true, complete and correct.

OTHER: I authorize investigation of my driver’s license history as a potential driver of company owned or leased vehicles. I also authorize the investigation of my personal background for any criminal history that may be in conflict with the company’s ability to provide good service to private as well as governmental entities. Personal background investigations will require fingerprinting of the employee during the employment period with CI.

I understand that any information provided by me that is found to be false, incomplete or misrepresented in any respect, will be sufficient cause to (A) eliminate me from further consideration for employment, or (B) may result in my immediate discharge from the employer’s service, whenever it is discovered.
